Emergency Service for clients who submitted an appliation in person. The ORG's Toronto Office as of July 30th will
be available at the new Downtown Toronto Service Ontario Centre located at 777 Bay Street, Lower Level.
Please remember to use only blue or black ink to complete the Statement of Live Birth. Should you make a mistake, simply stroke a line through the error. Both parents must initial the change. Do not use correction fluid or the form will be void. The Ingersoll Division Registrar will forward your newborn’s information to the Office of the Registrar General in Thunder Bay.
In approximately 10 – 12 weeks, you will receive a Notice of Registration from the Registrar General in Thunder Bay. This letter is to confirm that all the information submitted is correct. You may then request a birth certificate from the Office of the Registrar General. Requests for birth certificate forms are available from the Clerks Office or electronically at

If proof of birth for a newborn baby is required for travel purposes, prior to a Birth Certificate being available, a “Division Registrar’s Statement” is available for a fee of $25.00

To obtain a Birth Certificate, the application can be mailed or faxed to the Office of the Registrar General in Thunder Bay. It generally takes 6 – 8 weeks to obtain the Birth Certificate once the application has been mailed

If you require Emergency Services, please take the completed request, together with proof of urgency to the public counter in Toronto between 8:30 a.m. and 5:00 p.m. Monday to Friday. that the birth of every child born in the Province of Ontario must be registered with the municipal clerk of the municipality
in which the child was born, within 30 days of the date of birth.
Registration
To register, please complete the Statement of Live Birth received from the hospital. Forward the Statement of Live Birth,
together with $25.00 to the Town of Ingersoll, 130 Oxford Street 2nd Floor Ingersoll, Ontario, N5C 2V5. Please make cheques
payable to the “Town of Ingersoll”. In person, cash or debit are also accepted.
